Two boot disks differ by just that one line in config.sys. Why is it that when EMM386 is loaded, running Ghost is as much as 15% slower than when EMM386 is not loaded?

If it really is because of EMM386, then it may be a memory issue caused by it. One of EMM386's functions is to emulate contiguous extended memory XMS as segmented expanded memory EMS, and EMS is much slower than XMS.
Try adding the NOEMS parameter when loading EMM386, so that EMS is not used. If it's still slow then, it may be a problem with EMM386 itself.
