For example, EMM386.EX_ in the DOS 6.22 installation disk. I've tried numerous methods, but haven't been able to turn it into EMM386.EXE.

DOS6.22 already has expand.
expand emm386.ex_ c:\dos\emm386.exe
Extract the emm386.ex_ in the current directory to c:\dos\ with the extracted file name being emm386.exe.
