MB-721, the kind like the arcade machines on the street (6 buttons). Can it be driven under DOS?

I just went to GameTec's website and looked around. The documentation only says that MB-823 supports DOS and Windows; as for MB-721, I didn't see any mention of it. Later I found the driver, but it could only run under Windows.

I don't seem to have heard before that joysticks need drivers. You can just plug them directly into the serial port or the sound card and use them, except that usually the game itself comes with it.

That's right, for example the Super Nintendo emulator -- ZSNES comes with joystick drivers built in.
